How to claim
If you are an existing member and wish to make a claim, you can continue to do so in the normal way:
Ensure you keep the original receipts for any treatment you or your family pays for and return to us along with a completed claim form.
· STEP 1 Download a Claim Form.
· STEP 2 Fill in all the relevant sections carefully. Please note: missing information may delay the settlement of your claim.
· STEP 3 Sign the form – this is important as we are unable to deal with your claim without your signature.
· STEP 4 Send your completed Claim Form and original receipt(s) to:
