- Section A - details for you, your spouse or partner and any children. Select your level of cover and ensure you sign the declaration.
- Section B - medical information - click above to preview the questions.
- Section C - the Direct Debit mandate - click above to see a preview.
3. Sign the completed Application Form and Direct Debit mandate

Once your application has been processed you will receive a Welcome Pack and a certificate of cover through the post.

Changing levels can happen at any time before the age of 70 - it's very simple. Value members can upgrade to Classic or Advantage and Classic members can upgrade to Advantage, all of which will increase the benefits you receive.
Likewise, if your circumstances change you also have the option to decrease your cover.
